Who is chin reduction surgery or chin removal surgery for?

A large chin is one of three main chin categories. This surgery is intended for people who feel it is too large or asymmetrical. It can:

  • Reduce a large, prominent chin.
  • Reduce the size of a prominent, bulging, or overly protruding chin.
  • Straighten an asymmetrical chin that leans to one side.
  • Narrow a chin that is too wide or too square.
  • Correct a lower face that is too wide or too prominent by slimming it.

A cosmetic surgeon (maxillofacial, oral, or dental) can shorten (length) and slim (width) the lower face. The chin plays a very important aesthetic role because it defines the lower face and influences the lower lip, cheeks, and the skin of the neck below the jaw. When it is prominent, it can falsely give the impression of a dominant or stubborn person. The operation must be performed by an experienced plastic surgeon.

What surgeries are available to reduce chin size?

Some people with a very large chin may want to reduce it to achieve a more balanced face. To achieve this, reduction genioplasty improves facial harmony by softening the contours.
When undergoing surgery, the patient will consult a maxillofacial surgeon or an oral surgeon. The surgeon will operate under general anesthesia from the inside of the mouth. Consequently, there will be no scarring on the face. The surgeon will then cut the chin (osteotomy), remove a section, and then reattach it with titanium plates and screws. Alternatively, the maxillofacial surgeon may also shave the tip. Indeed, the bone and cartilage are worked using a milling machine or an ultrasonic (piezoelectric) device that can cut or grind.

It is possible to reduce the length and width of the chin. Reduction surgery has no impact on the position and function of the teeth.

The operation is performed as an outpatient procedure (without hospitalization) or with an overnight hospital stay. Fortunately, the operation is not very painful, but there will still be some slight swelling in the lower face and a slight loss of sensation, which is normally temporary. Afterward, the doctor will provide you with a prescription for painkillers and to apply cold to the lower face.

Procedure of the operation

  1. Patient’s consideration of whether or not to have their chin shortened

  3. The patient meets with the surgeon to confirm the feasibility of the operation. The surgeon presents the operation and its risks.
  4. Second consultation with the maxillofacial surgeon. Confirmation of the patient’s wishes.
  5. Pre-operative examinations are performed (blood tests, anesthesiologist’s assessment, facial X-rays, etc.). Alcohol and smoking cessation is required before the operation.
  6. Surgery: The surgeon makes incisions in the mouth, cuts or planes the bone and cartilage of the chin. He or she then places stitches inside the mouth. The operation lasts 1 to 2 hours.
  7. The patient leaves the same day or stays in the hospital for 1 day.
  8. Five days later, the compression bandage to prevent edema is removed.
  9. Seven days after the operation, the patient can resume a near-normal diet.
  10. Approximately 15 days later, the edema (swelling) and bruising in the lower part of the face have disappeared.
  11. Three months later, the bone has healed if osteosynthesis (bone cutting) was performed. Possibility of resuming sports.
  12. Six months later, the result is consolidated; normally, there will be no further changes.

Complications

The operation is generally low-risk, but like any surgery, there are risks:

  • Infections (need for mouthwashes and antibiotics).
  • Internal bleeding or bleeding in the mouth. If it is too severe, the surgeon may operate again immediately to stop the bleeding and drain it.
  • No bone regeneration (rare).
  • Temporary or permanent (rare) loss of sensation and tingling in the lower face.
  • Disappointing results: failed genioplasty, need for secondary genioplasty.
  • Difference between reduction genioplasty and “Long Face” surgery

During a chin reduction, the surgeon will only operate on the tip of the chin. Therefore, this procedure will be primarily cosmetic. Thus, the surgeon will, in a sense, shave off the tip of the chin. The ultimate goal is to achieve a more harmonious profile, with a chin that is less prominent and does not extend beyond the nose.

Long face surgery is more extensive. It involves the entire lower jaw, or even both jaws (bimaxillary surgery). This surgery aims to raise the entire lower part of the face and sometimes also move the chin back. It often corrects functional problems (difficulty closing the mouth, dental malocclusion, etc.).

Difference from skeletal class III malocclusion surgery

During skeletal class 3 malocclusion surgery, the maxillofacial surgeon will operate on the entire lower jaw and sometimes also the upper jaw. The goal of this operation is to move the lower jaw back and align the teeth so that the person can chew and eat normally.
